111學年度第2學期資格考試
請於口試前一個月提出申請，申請截止日05/26（五），口試請於07/14（五）前完成
**須修畢本所規定之應修學分與完成相關規定，使得申請考試，請自行確認**
1. 申請流程
線上申請：路徑【學生資訊系統→各項申請→博士學位候選人資格考試申請】
填寫論文題目、考試日期、考試地點、委員資料等
上傳文件：

• 指導教授推薦書-【表單下載】
• 博士資格考試-研究計畫書-【格式下載】
• 歷年成績單
  à送出申請
  列印：申請書－請檢查考試相關資料

2. 繳交資料至針灸研究所辦公室

• 資格考試申請檢核表-【表單下載】
• 紙本申請書（請指導教授簽名）
• 指導教授推薦書
• 博士資格考試-研究計畫書
• 歷年成績單

111 Academic Year 2nd Semester Application for PhD Candidacy Examination _2023
Application for PhD Candidacy Examination, please submit an application one month before the oral examination.
The application deadline is 05/26 (Fri), and the oral examination should be completed before 07/14 (Fri).
**Must complete the required credits and complete relevant regulations stipulated by the institute, so that you can apply for the exam, please confirm by yourself**

1. Application process
●Online application: Path [Student Information System→Various Applications→Application for Doctoral Candidate Qualifying Examination]
●Fill in the thesis title, exam date, exam location, committee member information, etc.
●Upload files:
(1) Letter of recommendation from the Adviser → [download the form]
(2) Research proposal→ [download the form]
(3) Transcripts over the years
●Send application
●Print: Application Form - Please check the relevant information of the examination

2. Submit the materials to the Office

●Letter of recommendation from the Adviser
